- All services require one valid, government‑issued photo ID (List A Document).
Must include your legal name, date of birth, photo, and signature. - All identification documents must be:
- Original (no photocopies, pictures, or scanned copies)
- Valid (current, unexpired, not punched)
- If your state ID or driver’s license is expired, you must bring:
- Your temporary ID/letter from the Department of Licensing (DOL)
- AND the punched/expired ID or a second form of photo identification
(Temporary ID is only valid when paired with one of these.)
FBI Channeler Services
- Require two forms of identification, including:
- One government‑issued photo ID
(e.g., driver’s license, passport, passport card, I‑551 Green Card) - One document showing your name and mailing address
(List A, B, or C)
- One government‑issued photo ID
- All documents must be:
- Original
- Valid
- Matching the name and address on your application

List B – Secondary Identification

- State‑issued birth certificate

- Native American tribal document
- Social Security card
- Certified court order (name change, gender change, adoption, divorce)
- Certified marriage certificate
- Consular Report of Birth Abroad
- Draft record
- School ID with photograph
- Certificate of Citizenship (N‑560)
- Replacement Certificate of Citizenship (N‑561)
- Certificate of Naturalization (N‑550)
- Replacement Certificate of Naturalization (N‑570)
List C – Supporting Documents
(Used to validate secondary identification)
At least two of the following current documents:
- Utility bill
- Voter registration card
- Vehicle registration/title
- Paycheck stub with name and address
