General Requirements

  • All services require one valid, government‑issued photo ID (List A Document).
    Must include your legal name, date of birth, photo, and signature.
  • All identification documents must be:
    • Original (no photocopies, pictures, or scanned copies)
    • Valid (current, unexpired, not punched)
  • If your state ID or driver’s license is expired, you must bring:
    • Your temporary ID/letter from the Department of Licensing (DOL)
    • AND the punched/expired ID or a second form of photo identification
      (Temporary ID is only valid when paired with one of these.)

FBI Channeler Services

  • Require two forms of identification, including:
    • One government‑issued photo ID
      (e.g., driver’s license, passport, passport card, I‑551 Green Card)
    • One document showing your name and mailing address
      (List A, B, or C)
  • All documents must be:
    • Original
    • Valid
    • Matching the name and address on your application

Tips & Best Practices

  • Requirements can vary—especially for out‑of‑state requests.
    Check instructions from your employer or requesting agency.
  • Best Practice: Bring two forms of ID
    • 1 List A + 1 List A/B/C
    • At least one must list your current mailing address
  • If you do not have a List A document:
    • Bring two List B + two List C documents
    • Ensure at least one includes your current mailing address

List B – Secondary Identification

(Two required if no List A is provided)

  • State‑issued birth certificate
  • U.S. Tribal or Bureau of Indian Affairs ID
  • Native American tribal document
  • Social Security card
  • Certified court order (name change, gender change, adoption, divorce)
  • Certified marriage certificate
  • Consular Report of Birth Abroad
  • Draft record
  • School ID with photograph
  • Certificate of Citizenship (N‑560)
  • Replacement Certificate of Citizenship (N‑561)
  • Certificate of Naturalization (N‑550)
  • Replacement Certificate of Naturalization (N‑570)

List C – Supporting Documents

(Used to validate secondary identification)

At least two of the following current documents:

  • Utility bill
  • Voter registration card
  • Vehicle registration/title
  • Paycheck stub with name and address
    (Financial info may be redacted)
  • Public assistance card
  • Notarized spouse/parent affidavit
